Summary:
Ageing
of population, growing mobility in society and growing
shortage of staff resources in the health care sector
require new models for information handling and
communication in order to guarantee quality-oriented health
care of the elderly. The project focuses on providing a
seamless and consistent information flow between home health
care and primary health care using mobile devices.
The
project aims at providing elderly people with improved
health care by development and evaluation of work-scenario
oriented IT support for health care personnel in home and
primary health care of the elderly, thereby addressing the
issues of
(1)
immediate and ubiquitous access to patient-oriented data in
specific work situations for different care providers,
(2)
decision support at the point of care and,
(3)
intuitive access to information services for the elderly and
their relatives.
In
order to fulfil its goal, the project is based on an unique
network of partners from community, county council and
industry as well as on established contacts with other
research projects in the field.
